Merchant Membership Form

Please use this form to enroll your business into the Clerk's “THANK A VET” Discount Program.  Be sure to include the official name of your business, its address, hours and discount specifications.  Your business information, website and logo will be displayed on www.erie.gov/clerk/thankavet and added to our business list which is handed to every veteran who registers for the "Thank A Vet" card.
