General Contractors must provide the following insurance: 

1. Workers Compensation (Form C105.2) – Accord forms are unacceptable
2. NYS Disability (Form DB120.2)

General Contractors that qualify for a Certificate of Attestation Exemption (CE-200) from New York State Workers Compensation and/or Disability Insurance Coverage must submit the CE-200 Exemption completed for the specific project and address on the permit application. 

General Contractors are cautioned to consult with an attorney to determine their obligation to provide proper insurance coverage or the Certificate of Attestation.
